You can configure cover sheets.

On the Forms Settings tab > Settings tab there is an option to add a cover sheet to all generated forms.

Click Edit and enter the form ID of the form you would like to use as a cover sheet. Then click Save. The form IDs can be found in the first column of the Form List screen.
